LIC Policy - Change of nomination?

This is a query about insurance in India.



I am having a couple of LIC policies. I need to change the nominee for each of them.



Please advice me on the procedure. I have downloaded Form No 3750 .



Do I need to visit the branch which has issued the policy? Or can agent proceed with my signed form with details?



Also is it necessary to submit the original policy to LIC



Please clarify.
