But if you meant the worst that could happen because of not having insurance:
1) You lose your license
2) They can take your car
3) You have to pay for all the damage to all the cars in the accident
4) Fines of several hundred dollars
5) You may have to pay a very large amount of money (perhaps ten million dollars) to anyone injured in the accident
6) Possible jail or prison, depending on state law
If you are stopped by a policeman they will ask for your registration and insurance.
If you cannot provide insurance, they will park your car on the side of the road and take you straight to jail. Wear your seatbelt and drive slow.
If you are in an accident. Again, you will go to jail for not having any insurance.
They will sue you for damages. They can garnish your wages and tap into your savings/checking

I was in an accident years ago. By mistake I gave the policeman an expired insurance card. He came back to the car with his handcuffs ready to put on me if I did not show a current insurance card. Some states may be different than others
Probably worst case scenario would be you put somebody into a wheelchair for life. They will garnish 30% of your wages before taxes for as long as you work. This means if you make $15 per hour they'll take $4.50 off the top before taxes and give it to the person you maimed. You'll then pay taxes on the $15.
I agree with the other answers.
You haven't asked about the worst case scenario for the innocent party if you have an accident.
At best they will have an expensive car bill at worst crippled and unable to work.
It's your conscience. Think about it.
If you can't afford car insurance then you can't afford the car. It's a package deal. PERIOD.
